Service Description

Transitioning into or diverting from a nursing home can be a challenging process, but A1 Big Apple Coordin is here to make it easier. Our Nursing Home Transition & Diversion program, funded by Medicaid, offers support and services to seniors and individuals with disabilities who prefer to stay in or return to their homes. We believe that everyone has the right to live where they choose, and our program helps make that a reality.
